Dabigatran is an oral direct thrombin (factor IIa) inhibitor used as an anticoagulant. It works by selectively and reversibly blocking thrombin, preventing the conversion of fibrinogen to fibrin and thereby reducing blood clot formation. Dabigatran is commonly prescribed for stroke prevention in patients with nonvalvular atrial fibrillation and for the treatment and prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE). It is administered as the prodrug dabigatran etexilate and does not require routine coagulation monitoring. An approved reversal agent (idarucizumab) is available for emergency situations.
