Grass pollen season arrives every year, and so do the same patients presenting with all-too-familiar symptoms like congestion, sneezing, and itchy eyes. Treatment is started, adjusted, and often repeated, but many return still symptomatic and without a clear understanding of what is driving their condition.

For primary care providers, this pattern is common. But it raises an important question:

Grass pollen allergies are often assumed based on seasonality. However, overlapping symptoms, multiple sensitizations, and variable exposure make the underlying trigger not always clear.1  Without diagnostic clarity, care can become reactive, leading to unnecessary repeat visits and ongoing patient frustration.

If symptoms persist despite treatment, consider reassessing the diagnosis, adherence, and trigger exposure. Confirm that the patient is using medications correctly and consistently, and review ongoing allergen exposure, including seasonal and environmental factors. Step up therapy as appropriate or consider referral to an allergy specialist for further evaluation.

These small actions can help improve symptom control and quality of life.

Refer patients to an allergy specialist if symptoms persist despite treatment or the diagnosis is unclear. Referral is also advised when considering allergen immunotherapy, since specialist assessment is necessary to evaluate appropriateness and guide therapy. Patients with severe or difficult-to-control asthma, especially if an allergic trigger is suspected, may benefit from specialist care. Additionally, refer patients with sensitization to multiple allergens, as further detailed evaluation can identify relevant triggers and enable more targeted management.
