Digital PCR in detection of clonal disease evolution in childhood acute lymphoblastic leukemiaDr. Prateek BhatiaB-ALL genomics displays heterogeneity with primary genetic abnormalities followed by subsequent driving lesions influencing treatment response and leukemogenesis. Although some cases experience disease relapse, recent research has identified recurrent RAS, NT5C2, PMS2, TP53, and KMT2D clonal mutations in pediatric B-ALL. The "DETECTOR study" aims to validate clonal trajectories to relapse using a digital PCR-based validation study. The results of the study confidently showed significant and minor clones driving early relapses, indicating the utility of digital PCR for clonal monitoring in LMIC regions. These findings will enable the clinical implementation of clone monitoring or early clone detection tests during therapy to predict and detect relapses before their clinical manifestation, hence improving outcomes.20 mins
